Before you can apply for Student Disability Services

  1. Apply to the college
  2. See a counselor for an Ed Plan to choose courses
  3. Register for courses.
  4. Now you can proceed to apply to our program.

To Apply for the Program

You will need your CHC ID number and CHC Student Email Address.

Apply To Crafton

Submit the SAS application form if you are NEW to the program.  
Returning students just need to contact our office.                            
  1. Application for SAS Services
  2. Consent for Release of SAS Information
  3. Students Rights and Responsibilities
  4. Verification of Student Disability (This is not needed if student has a 504 Plan or an IEP AND a PsychEd Report)
